Description:The purpose of this initiative is to support a cooperative agreement entity – an Implementation and Technical Assistance Center (ITAC) – that will facilitate the adaptation and implementation of previously evaluated housing interventions to address housing needs for youth and young adults, men who have sex with men, transgender, and justice involved people with HIV. The initiative will fund up to 12 implementation sites. This initiative will draw upon the experience and lessons learned from three (3) prior RWHAP Special Projects of National Significance (SPNS) housing-related initiatives as the source of evidence-informed interventions to be implemented and adapted: 1) Building a Medical Home for Multiply Diagnosed HIV Positive Homeless Populations 2) Addressing HIV Care and Housing Coordination through Data Integration 3) Improving HIV Health Outcomes through the Coordination of Supportive Employment and Housing Services
