Description:The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $30,000,000 for Year 1, subject to the availability of funds. This NOFO is to support implementation of comprehensive HIV prevention, care, and treatment programs for HIV epidemic control and support for other prevention, detection, and control of infectious diseases and non-infectious disease threats in Kampala Region (Kampala and Wakiso districts and all associated municipalities). This will entail implementation of HIV services including HIV case finding especially among men, children and key and priority populations; HIV care and treatment; TB prevention, diagnosis, and treatment; elimination of mother-to-child transmission; management of advanced HIV disease and integration of non-communicable diseases; voluntary medical male circumcision; pre-exposure prophylaxis, and related laboratory and strategic information services, including enhanced surveillance to understand where incident cases are occurring, through a health systems strengthening approach. The successful applicant will work closely with the Ministry of Health to strengthen the technical capacity of Regional Referral Hospitals and District Health Teams for effective and sustainable decentralized response and will work with faith- and community-based partners to improve client-centered service delivery. Expected outcomes include improved access, quality and coverage of HIV services ensuring 95% of people living with HIV know their status; 95% of people diagnosed receive antiretroviral therapy; and 95% of those on treatment are virally suppressed.
