Description: |
No Wrong Door (NWD) Systems success hinges on the ability of the state to convene and establish an on-going governance structure. Currently, over half of states (28) have no fully functioning governing body for all populations. The purpose of this Funding Opportunity Announcement (FoA) is to provide innovation and infrastructure grants to support 10-15 states for two years to develop and enhance a state governance and administration structure to coordinate, strengthen and streamline access to home and community-based services and supports. In addition, explore opportunities to collaborate with state leaders across Medicaid, nutrition, housing, transportation, and employment to proactively and strategically align state resources to address social needs. NWD Governance and Administration involves collaborative efforts among multiple state agencies (e.g., state Medicaid; State Unit on Aging; state agencies that serve or represent interest of individuals with physical, intellectual and developmental disabilities; and state authorities administering mental and behavioral health services); The goals of the visioning sessions process include: 1.Establishing a meaningful governing body for the state NWD System in line with NWD Key Elements; 2.Identifying a lead agency; 3.Developing a plan to improve (IT systems, PCC, streamlined access and public outreach); 4.Develop a Disparity Impact Statement to address gaps in populations served and/or key stakeholders missing and developing a plan for how to engage; 5.Establishing performance standards and quality improvement strategies; 6.Examining the current system for person-centered practices gaps 7.Developing work plans for each NWD function |
